Founded in 2004, the youth residential rehabilitation service uses community-based care in a small-group home. Each client has an individual functional service plan prepared with a multidisciplinary team.
Supported flats and a four-person community home help young people move towards independent living. Support is also offered in a young person's own home through daytime activities and home visits. A self-monitoring plan guides operations, with an annual client survey supporting development. A toilet is available.
