Founded in 2004, this service provides residential rehabilitation and supported housing for young people. The small-group home follows community-based care principles, and an individual functional service plan is drawn up for each young person with a multi-professional team. The service is meant for young people who need mental health rehabilitation.
For those moving toward independent living there are supported flats in the city centre and in Kuhasalo, and a group-based supported living home for four people. Support is also given in a young person's own home, including a daytime activity programme and home visits. Operations are developed using an annual client satisfaction survey, and community and supported living follow a self-monitoring plan. The premises have an accessible toilet.
