Founded in 2004, the service provides residential rehabilitation and supported housing for young people with mental health rehabilitation needs. The small-group home follows community-based care principles. Each resident receives an individual functional service plan drawn up with a multidisciplinary team.
Independent living is supported through separate apartments and a four-person community home. Home visits and daytime activities also support young people in their own homes. Operations use a self-monitoring plan and an annual client survey. An accessible toilet is available.
As a client or relative, share your experience of the housing support in the comments for others considering the service.
