Founded in 2004, the service provides residential rehabilitation and supported housing for young people with mental health rehabilitation needs. The small-group home follows community-based care principles, with an individual functional service plan prepared by a multidisciplinary team.
Support towards independent living is offered in supported apartments and a four-person community home. Home visits and daytime activities can also support young people in their own homes. Annual client surveys inform development, and a self-monitoring plan is in place. There is an accessible toilet.
If you wish, share your supported housing experience below to help other young people and relatives.
